Bedside signing & seal

Valid photo ID
Every signer needs a current government-issued photo ID — California driver's license, state ID, U.S. passport, or equivalent. If the patient's ID is at home, have a family member bring it before the appointment.

Bring complete documents — not just the signature page. If you need documents generated on-site, we bring a laptop, portable printer, and internet connection to produce forms during our visit.
Any required witnesses

Visitor parking is available on campus and street parking is accessible on Vermont Avenue. We allow extra time for parking and will text you when we are on-site.
