Hospital Bedside Notary — Boyle Heights
When a patient at Keck Hospital of USC needs documents notarized, we come to the bedside — powers of attorney, advance healthcare directives, living wills, and more. Serving all inpatient units on the USC Health Sciences Campus.
We are an independent mobile notary service — not affiliated with Keck Hospital of USC. We come directly to the patient's bedside in any unit the hospital permits visitors to enter. Call or text (213) 933‑2507 and let us know the patient's room number and what needs to be signed.

Located at 1500 San Pablo Street, Keck Hospital is distinct from LA General Medical Center nearby. The campus is home to complex transplant, oncology, and neurology programs. Parking is available in the South San Pablo Structure and Valley Lot with campus shuttles. Travel time from Granada Hills typically ranges 40–60 minutes depending on traffic on the 5, 10, and 101 freeways.
We check in with the nursing staff, bring all equipment to the patient room, verify identity, and notarize the documents. Most visits take 20–45 minutes depending on the number of documents.
Hospital Documents We Notarize Bedside
Click any document type below for complete information on what is required, accepted, and how notarization works for that specific form.
Before your appointment
Every signer needs a current government-issued photo ID — California driver's license, state ID, U.S. passport, or equivalent. If the patient's ID is at home, have a family member bring it before the appointment.
Bring complete documents — not just the signature page. If you need documents generated on-site, we bring a laptop, portable printer, and internet connection to produce forms during our visit.
Healthcare directives typically require two witnesses who are not named in the document and are not employees of the treating facility. Confirm witness requirements for your specific document before we arrive.
