About Los Robles
Los Robles Regional Medical Center is the only Level II Trauma Center in eastern Ventura County, located in Thousand Oaks. Families throughout the Conejo Valley call us when a patient cannot travel to a notary office. We travel via the 101 freeway with our seal, laptop, and portable printer, and can coordinate with outside attorneys to print and organize documents before signing.

Located at 215 West Janss Road in Thousand Oaks (Ventura County), Los Robles has free parking on campus. Visiting hours are Monday through Friday 8:00 AM – 8:00 PM, with extended hours for L&D, hospice, pediatric/NICU, and special needs patients. Travel time from Granada Hills via the 101 freeway is typically 40–60 minutes in normal traffic; rush hour through Calabasas and Agoura Hills can take longer.
We check in with the nursing staff, bring all equipment to the patient room, verify identity, and notarize the documents. Most visits take 20–45 minutes depending on the number of documents.

Before your appointment
Every signer needs a current government-issued photo ID — California driver's license, state ID, U.S. passport, or equivalent. If the patient's ID is at home, have a family member bring it before the appointment.
Bring complete documents — not just the signature page. If you need documents generated on-site, we bring a laptop, portable printer, and internet connection to produce forms during our visit.
Healthcare directives typically require two witnesses who are not named in the document and are not employees of the treating facility. Confirm witness requirements for your specific document before we arrive.
